Right to Receive Good Faith Estimate

You have the right to receive a “Good Faith Estimate”: explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other      provider you choose, for a Good Faith Estimate before you schedule an item or service.
  • If you received a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
  • Make sure to save a copy or a picture of your Good Faith Estimate

To request your Good Faith Estimate, please contact your provider directly to establish fee per hour of service in email form for your records.

This Privacy Policy describes how Reflective Artistry Studio and Counseling, LLC (the "Site", "we", "us", "our” collects, uses, and discloses your personal information when you access our website, use our services, or otherwise interact with our website or otherwise communicate with us (collectively, the "Services"). For purposes of this Privacy Policy, "you" and ""your" means you as the user of the Services, whether you are a client/patient, website visitor, or another individual whose information we have collected pursuant to this Privacy Policy.

Please read this Privacy Policy carefully. By using and accessing any of the Services, you agree to the collection, use, and disclosure of your information as described in this Privacy Policy. If you do not agree to this Privacy Policy, please do not use or access any of the Services. The terms in this policy refer specifically to this website and not to third party links, applications, or forms found on this website.

We may update this Privacy Policy from time to time, including to reflect changes to our practices or for other operational, legal, or regulatory reasons. We will post the revised Privacy Policy on the Site, update the "Last updated" date and take any other steps required by applicable law.

The information that we collect, and use varies depending on how you interact with us.  Additionally, the personal information you enter into Acuity and/or Thera nest (our client scheduling portal), maybe collected in order to assist in scheduling.

In addition to the specific uses set out below, we may use information we collect about you to communicate with you, provide the Services, comply with any applicable legal obligations, enforce any applicable terms of service, and to protect or defend the Services, our rights, and the rights of our users or others.

The types of personal information we obtain about you depends on how you interact with our Site and use our Services. When we use the term "personal information", we are referring to information that identifies, relates to, describes or can be associated with you. The following sections describe the categories and specific types of personal information we collect.

Information We Collect Directly from You

Information that you directly submit to us through our Services may include:

  • Basic contact details including your name, address, phone number, email.
  • Customer support information including the information you choose to include in communications with us, for example, when sending a message through the Services or through Thera nest system for scheduling.

Some features of the Services may require you to directly provide us with certain information about yourself. You may elect not to provide this information, but doing so may prevent you from using or accessing these features.

Your information, including personal data collected through the company website, is processed at the company's operating offices and in any other places where the parties involved in the processing are located.   • With your consent: We may disclose your personal and protected health information with your consent as indicated by you on the form for authorization for disclosure and release of confidential information embedded on our client forms page.
  • For continuity of care: We may provide your information to government bodies or licensure boards for HIPAA-compliancy audit per the limitations and guidelines outlined in HIPAA law and detailed in our HIPAA Notice of Privacy Policy Practices and business partners that provide services and products under the required HIPAA-compliant business associate agreement to protect your privacy; or to a records custodian in the event of our incapacitation or death.
  • To manage your requests: To attend and manage your requests to us.
  • To facilitate video conferencing with other users under the conditions of your informed consent agreement for group therapy.
  • To comply with law enforcement: To protect and defend the rights or property of the company; prevent or investigate possible wrongdoing in connection with the service; protect the personal safety of users of the service or the public; or protect against legal liability. 
  • When you voluntarily choose to interact in public areas with other users, directly or indirectly, such as sites associated with this company and embedded on this website. 
  • For other purposes: We may use your information for other purposes, such as data analysis, identifying usage trends, and to evaluate and improve our service, products, services, marketing and your experience.

Cookies

Like many websites, Cookies may be used on our Site. RASC does not use Cookies for any purposes whatsoever, however, our web hosting provider, GoDaddy, may use basic web traffic data generated by your use of our site/services. Such use is not at the request of, nor of any benefit to, RASC. 

Most browsers automatically accept Cookies by default, but you can choose to set your browser to remove or reject Cookies through your browser controls. Please keep in mind that removing or blocking Cookies can negatively impact your user experience and may cause some of the Services, including certain features and general functionality, to work incorrectly or no longer be available. Additionally, blocking Cookies may not completely prevent how your information is shared by our web hosting platform.


Disclosure of Personal Information

We do not disclose your personal information to third parties unless such disclosure is mandated by law, regulation or court order. Such disclosures are subject to this Privacy Policy. Such circumstances may include:

  • With third parties who perform services on our behalf (e.g., IT management, payment processing, data analytics).

Security and Retention of Your Information

Please be aware that no security measures are perfect or impenetrable, and we cannot guarantee “perfect security.” In addition, any information you send to us may not be secure while in transit. While we strive to use commercially acceptable means to protect your personal data, we cannot guarantee its absolute security. In the event of a breach, we will notify you of the breach. If such breach involves protected health information, we will comply with the breach notification rule to notify you and the Department of Health and Human Services Secretary that a breach has occurred. 

How long we retain your personal information depends on different factors, such as whether we need the information to maintain your account, to provide the Services, comply with legal obligations, resolve disputes or enforce other applicable contracts and policies.
 

All data and information retention are subject to relevant and controlling HIPAA regulations.
